Short concise referrals appreciated.

Stating:

  • Required test
  • Reason for test
  • Brief relevant medical history
  • Current medications (for reviewing Cardiologist to view)
  • Basic patient information (Name, DOB, NHI, address, Phone number and email address)
  • Southern Cross member?

Results

Securely emailed back to referral practitioner through Healthlink or via referrers practice admin email in PDF format.



Results provided within 3days of completing test.

Appointments

Usually, the following week of receiving referral.


Once referral received, CardioLabs will call patient to arrange suitable time and if appropriate, seek Southern Cross approval.


Tuesday clinics in Waikanae Health Specialist Centre, Waikanae.


Friday clinics at Ropata Witako Specialists, Lower Hutt.
